PROTOCOL FOR COACHES AND PARENTS WHEN PLAYERS EXHIBIT SIGNS, SYMPTOMS, OR BEHAVIORS CONSISTENT WITH A CONCUSSION DURING PRACTICE OR GAMES

1. Remove any player immediately that shows signs, symptoms, or behaviors consistent with a concussion from the activity or competition.

The parent/guardian shall have the player examined, evaluated and eventually cleared if warranted by a qualified health care provider, licensed medical doctor (M.D.), Osteopathic Physician (D.O.) , Certified Athletic Trainer, Certified Nurse Practitioner, Physician's Assistant, Osteopathic Physician or a Clinical Neuropsychologist with Concussion Training.

3. If it is determined the player has sustained a concussion, the health care provider examining the player must complete and sign the "Concussion Return to Play" form. The form must then be given, faxed, or emailed by the parents to Longmont Baseball League office. The League office will then notify the Head coach as to when the player may return to practices or games.

4. If it is determined the player has not sustained a concussion, the player may return to the activity or competition. After the health care provider examining the player completes and signs the "Concussion Return to Play" form, the form must then be given by the parents to the Longmont Baseball League office. The League office will then notify the Head coach that the player has been cleared to participate in practices and games.
